We do not make or change medication plans. Instead, our caregivers follow the schedule that has been set by your loved one’s physician, nurse, or family.

Reviewing the existing medication schedule with the family and the Care Manager.

Knowing what time each dose is supposed to be taken and the food or sleeping conditions.

Offering friendly and timely reminders when it’s time to take medication.

Support With Pill Organizers

Pill organizers can be very helpful, but they only work if they are used correctly and consistently. While the family or a licensed professional is responsible for filling the organizer, our caregivers can:

Remind your loved one to take the pills from the correct day and time slot.

Place the organizer in a visible and easy- to-reach location.

When needed, help your loved one open containers or blister packs.

Notice when sections are still full at the end of the day and mention this to the Care Manager or family.

Watching for Patterns and Communicating Concerns

Our caregivers are keeping an eye at all times to make sure your loved one is doing great. If they notice skipped, delayed, or refused doses, the care manager and the family will be informed immediately.

Comments regarding a dislike of certain pills will be communicated. The Care Manager may suggest a follow – up with the prescribing provider.

Clear Non – Medical Boundaries

It is crucial to understand that our caregivers and Preserve Home Care do not replace medical institutions or staff, we help as much as we can within our non – medical boundaries.

Our Caregivers Can:

Provide reminders and motivation.

Assist with opening containers and accessing pill organizers.

Observe and report concerns to the Care Manager and family.

They do not:

Administer injections.

Decide which medications should be taken or stopped.

Change dosages.

Perform any clinical tasks that must be done by a licensed nurse or physician.
