We know that every senior is unique; each has a different story, health, history, home environment, and set of priorities. For these reasons, our care plans are designed specifically for an individual client. Preserve Home Care won’t offer you a generic schedule.
Customized home care plans are the foundation of everything we do. We serve Beverly Hills, Westside Los Angeles, Palm Springs, and the Coachella Valley, and for every family we work with, we begin by listening.

Your Initial Consultation and In-Home Care Assessment
The first setp is a personalized in-home care assessment, or if needed, a virtual visit. This is the stage where we get to know your senior relative and learn more about their everyday lives or routines.
During this consultation, your Care Manager will pay close attention to your concerns and goals for the care. Always keeping the non-medical limits, the medical conditions and current support will also be reviewed.
They will also ask you about your loved one’s routine, sleep patterns, energy levels, and usual mood. Their personal preferences matter too, including cultural or religious considerations.
The Care Manager will also walk through the home to notice safety concerns and practical needs when the visit is in person. Remember, this is a conversation, not an interrogation. We want your loved one to feel included and respected, taking the family into consideration.
Designing a Personalized Senior Care Plan
The Care Manager will design a personalized senior care plan following what you shared during the assessment. This plan is the guide for everyday care at home.
It includes the services required for your loved one, such as personal care, companionship, housekeeping, transportation, or specialized support. The schedule will also be defined, stating how often caregivers should visit, for how long, and the best time of day for their support.
The goals for care will also be clearly defined, ensuring the best possible attention for the senior. We review the plan with you, answer questions, and make sure it feels realistic for your loved one and for your family.
Matching Caregivers and Setting Up the Schedule
Once the care plan is in place, we move to caregiver matching and scheduling. This is where the plan becomes real support in the home. The Care Manager’s task is to find the most accurate match of the caregiver’s skills with your loved one’s needs.
Aside from skills, personality, temperament, language, and cultural fit are also taken into account. After the caregiver is selected, a schedule and consistent routine that everyone can work with is discussed. Finally, we introduce the caregiver and set expectations so the first visits feel as smooth as possible.
Our goal is to match you with caregivers who feel like a natural extension of your support system, not strangers rotating in and out of your home.
Ongoing Monitoring and Adjustments
Your Care Manager will be actively involved in your care, remembering and following your loved one’s changing needs. This ongoing support includes regular check-ins with caregivers to follow the state of visits, aswell as periodic conversations with the client and family for feedback.
As time passes by, your senior relative will need different things, so adjustments to tasks and schedules will need to be made, not just because of aging, but also some events, such as a fall, a hospital stay, or a new diagnosis.
This continuous monitoring keeps your customized home care plan current, so it always reflects what is happening at the moment, not just what was true at the beginning.
Family Communication and Transparency
Preserve Home Care keeps transparency about what is happening in your home, being clear about everything. We emphasize clear, respectful communication with families, at a pace and level of detail that works for you.
Your Care Manager will keep you updated about how your loved one is managing daily routines and share observations from the caregiver about their mood, appetite, energy, or safety concerns.
They will also help you prepare questions for doctors or therapists based on what we see at home and discuss the projected care plan changes before they are implemented. This transparency strengthens trust and helps everyone work toward the same goals.
